The Personnel Action Request form is to be filled out by the supervisor.
All new employees must go through Martha Martinez, Human Resources. Each employee must have a Personnel Action Request form submitted by the hiring supervisor, approved by cabinet, and required documents submitted prior to starting work.
A Personnel Action Request form must be submitted to hire, change positions, change hours, change pay or do additional work outside the already assigned hours.
The Personnel Action Request initiates the changes and must be submitted and approved prior to the employee beginning work. Upon final approval of the Personnel Action Request form employees may initiate work.
Supervisors must submit a Personnel Action Request form for part time/as needed employees at the start of each fiscal year or prior.
